More than half of Americans live with one or more chronic diseases, such as heart disease, stroke, diabetes, cancer and asthma. Chronic disease impacts quality of lives and drives up healthcare costs.
Mercy’s commitment to battling chronic disease is reflected in its partnership with Heritage Area Agency on Aging for a Chronic Disease Self Management Program (CDSMP). This year, the program was selected as one of 20 programs (from a total of 600 CDSMPs) selected to participate in a national study. Mercy is the only site from Iowa.
The national study will examine programmatic impacts and is sponsored by the National Council on Aging and it will be conducted by Texas A&M Health Science Center and Stanford University.
